Requirements for Liberians No Longer Holding Liberian Citizenship
Please be informed that if you are a Liberian living in the United States and you have become a US citizen, you are required to obtain a Liberian visa from the Embassy of Liberia in Washington, D.C.or The New York Consolate, prior to travel to Liberia. If you were born in Liberia and once a Liberian citizen, you will need a visa to travel to Liberia, if you are holding a U.S. Passport..
Airlines by regulation, will not allow you to board if you do not possess a visa to enter Liberia with a US passport. Please take note of this notice and govern yourselves accordingly, to avoid any embarrassment from the airlines and undue delays.

In accordance with International Health Regulations, all persons entering Liberia are required to have a Valid Certificate of Immunization against Yellow Fever.

All fees are to be paid in U.S. Dollars via Money Orders, Cashier Checks or Bank drafts and made payable to Embassy of Liberia/Consular Services. Personal checks are not acceptable. ABSOLUTELY NO CASH will be accepted as payment as of Monday, April 14, 2008.

b. Two (2) passport size photographs.(FULL Face view on a White background)
c. Medical Certificate from a recognized physician indicating that the applicant in good health and free from all communicable diseases.
d. Application for visa to be completed in duplicate.
e. Evidence of financial support.
f. Letter requesting for visa indicating place of accommodation in Liberia.
g. Self addressed prepaid envelope (overnight) for return of passport
h. Visa will only be issued Two Weeks to the Expected Travel Time.

REQUIREMENTS FOR AFFIDAVIT OF BIRTH
1. A Birth Affidavit is issued to Liberian Citizens who have lost or misplaced their original Birth Certificate.
2. Applicants must show evidence of Liberian citizenship such as:
a. Photocopy of missing Birth Certificate; or
b. Original Liberian Nationalization Certificate
3. If the above request is not obtainable, then the following must be submitted:
a. notarized letter addressed to the Consular Section, stating how the original
Birth Certificate got missing.
b. Three (3) notarized letters from other Liberian Citizens testifying to the
Citizenship of the applicant and photocopies of the first five pages of the
applicant's Liberian Passport.Letters should be addressed to the Consular
Section.
4.Two (2) passport size photographs (Full Face View on a white background).
5. Seventy-Five ($75.00) Money Order or Cashier Check made payable to the Embassy of Liberia.
6. A completed application form.
7. Prepaid self-addressed Express Mail/FedEx envelope to return your document.
8. Interview by the Consular Section.

Birth Affidavit
Travel Document (Laissez Passez)
A Travel Document is issued to Liberian Citizens who have lost their original passports and those holding a valid Liberian Laissez Passez.
1. Applicants must show evidence of Liberian citizenship such as:
a. Photocopy of missing passport.
b. Original and photocopy of Liberian National Identification Card.
c. Original copy of Liberian Naturalization Certificate.
2. If the documents requested are not obtainable, then the following must be submitted:
a. A notarized letter addressed to the Consular Section, stating how the origina passport; a Laissez Passez got missing.
b. Three (3) notarized letters from Liberian citizens addressed to the Consular Section testifying to the citizenship claim of the applicant, along with photocopies of the first five pages of their Liberian passports.
3. Two (2) passport size photographs (Full Face View on a white background).
4. Seventy-Five dollars ($75.00) Money Order made payable to the Embassy of Liberia.
5. Completed application form.
6. Interview by the Consular Section

1. Mail in the expired Liberian Passport.(with 2 Passport Size Photos FULL FACE
VIEW, and a filled out "Renewal form")
2.Seventy-Five Dollars($75.00) Money Order or Cashier Check made payable to the Embassy of Liberia.
3. Prepaid self-addressed Express Mail/FedEx envelope to return your document.
4. You may also bring these items to the Embassy for the renewal of your Passport.

1. Letter from Funeral Home indicating arrangements for the transfer of Remains.
2. Embalming Affidavit.
3. Burial Transit Permit.
4. Certified Copy of Death Certificate.
5. Non-Contagious Disease Certificate.
6. Passport or Alien Card of The Deceased.
7. Non-Contraband Letter.
8. Flight Information-(Name of Airline, Flight #, Date & Time of Departure/Arrival,
Name, Address & Telephone # of Shipping Funeral Home; and Name, Address &
Telephone # of Receiving Funeral Home).
